Another way these unnecessary costs are passed on to the insured is that they can quickly max out your insurance and you don’t find out until after the test has been done if the coverage will be denied due to some technicality.  Yes, it has been proven that health insurance companies constructively look for ways to not pay a medical claim, especially with expensive medical tests.  Another thing is that if you actually ask your doctor or hospital the actual price of the test they usually don’t know.  They actually do know and don’t want to scare you away from these very costly, unnecessary and profitable tests.
